چکیده
Prosthetic valves may make ventricular access difficult in radiofrequency catheter ablation. This case report describes an optimized atrial transseptal approach to access the ventricle in a patient with left ventricular tachycardia and a mechanical prosthetic aortic valve.

Unconventional warfare: Successful ablation of ventricular tachycardia by direct ventricular puncture in a patient with double mechanical heart valves
Introduction Catheter ablation of ventricular tachycardia (VT) has improved dramatically over the last 2 decades, with the introduction of new mapping and ablation technologies. The use of epicardial access and transcoronary ethanol ablation (TCEA) has been an important innovation to target arrhythmogenic substrate that is not accessible from the standard endocardial approach. Mechanical heart ...

patients tolerated the procedure well, with minimal discomfort. In the retrograde aortic group, I patient had a large hematoma at the site of the arterial puncture, and another had ventricular fibrillation related to a sudden impedance increase during the successful radiofre-quency ablation attempt. After ablation, patients were treated with aspirin (325 mglday for 1 month). Radiofrequency abla...

OBJECTIVE To report retrospectively on the training and subsequent experience of two operators in transseptal ablation of arrhythmias arising in the left atrium and left atrioventricular annulus, to show whether, with adequate training and careful attention to detail, this is a safe and effective technique. SETTING Electrophysiological studies and transseptal procedures were performed in the ...

Letter by Yamada and Kay Regarding Article, “Ventricular Arrhythmias Arising From the Left Ventricular Outflow Tract Below the Aortic Sinus Cusps: Mapping and Catheter Ablation via Transseptal Approach and Electrocardiographic Characteristics” We read with great interest the most recent publication by Dr Ouyang et al on the transseptal approach in catheter ablation of ventricular arrhythmias (V...
متن کاملEpicardial catheter ablation of ventricular tachycardia in no entry left ventricle: mechanical aortic and mitral valves.
BACKGROUND In patients with mechanical aortic and mitral valves and left ventricular tachycardia, catheter ablation may be prevented by limited access to the left ventricle. METHODS AND RESULTS In our series of 6 patients, 2 patients underwent direct surgical ablation and 4 underwent epicardial catheter ablation via a pericardial window. All patients had abnormal low voltage areas with fracti...
